This is where you can find the name or location of a provider or hospital.

Instructions:
Search by last name. Fill in the last name of the provider or hospital you are looking for.

Search by location. Fill in a ZIP code. To search in a general area, members can enter the ZIP code and select how many miles around the ZIP code you would like to search. You can also search by city.

Search by specialty. Use the drop down box to find a doctor by specialty. For Eye Care, click here.

Please remember:

Members need a referral from their Primary Care Provider (PCP) to visit most specialists.
Members need to Golden Advantage Plus Member Services at
(800) 953-8854 before seeing a new PCP.

For routine eye care services, click on BLOCK VISION to find the eye care doctor nearest to you. Or simply call (800) 428-8789 to speak with someone who will help you with your eye care questions.

Eye care services are available to both adults and children:

Under age 21 - one eye exam, one pair of lenses and one new eyeglass frame each year.
21 and over - one eye exam, one pair of lenses each year, and one new eyeglass frame every two years.
