We are pleased to offer our newest health plan, Golden Advantage Plus, for those who are
enrolled in both Medicare and Medicaid and live in Garrett, Allegany and Washington counties. Golden Advantage Plus pays for the following benefits
NOT paid for by Medicare:
• Preventive dental services (2 cleanings per year)
• Routine hearing tests (1 per year)
• Hearing aids (up to $750 per year)
• Routine eye exam (1 per year)
• Lens and eyeglass frames ($100 per year)
• Routine chiropractic care (12 visits per year)
• Routine physical exam (1 per year)

2008 Golden Advantage Plus Plan Documents
Golden Advantage Plus enrolls all plan members into Case Management. These services are provided to facilitate
access to medical care services and benefits offered by the plan. Our Case Management Department is available
by phone, Monday through Friday during normal business hours. Call (866) 651-7843 or TTY/TDD (800) 735-2258
and ask to speak with the Case Management Department.
Golden Advantage Plus has a contract with the Centers for Medicare and Medicaid (CMS).
This contract renews each year. At the end of the year, the contract is reviewed and either
Golden Advantage Plus or CMS can decide to end it. If this happens, you will receive a 90 day advance notice.
